What is DMSA Scan?

DMSA stands for Dimercapto Succinic Acid. It is a radioactive tracer (radioisotope) injected into the patient’s body to detect improper functioning of the kidneys under the gamma scanner.

DMSA Scan helps to evaluate the size, shape, and position of the kidney. The tracer highlights the blood vessels and enables a scanner to create detailed images of the inner structure of the kidney.

Although a bit different from traditional X-Rays and CT or MRI scans, nuclear medicine tests using radioisotopes like DMSA have the same amount of radiation as them. Investigations like ultrasound scan cannot show the working of kidneys. Whereas DMSA Scan has more chances of getting accurate results.

Who is recommended for DMSA Scan?

DMSA scan is used to check the structure of the kidneys, their size, and shape. It is commonly recommended for younger children who may have UTI (Urinary Tract Infections).

It shows the improper functioning or scarring of kidneys. Scarring is generally caused when the flowing urine travels back from the bladder to the kidneys. This condition is called vesicoureteric reflux. DMSA scans can easily look for damage following an injury or reduced blood supply to the kidneys.

A DMSA scan enables a doctor to see the functioning tissue of your kidneys or to evaluate any inflammation. It helps your physician to compare the function of each kidney to see if one kidney functions differently to the other.

What are the preparations required for a DMSA test?

Before the test begins you must clear all your queries from your concerned doctor or the radiologist who will do your test. They may inform you of the following points:

  • Wear loose and comfortable clothes on the day of the test. Probably the lab will provide you a loose hospital gown to wear.
  • You may be asked to provide a urine sample to make sure you do not have an active urine infection at the time of the test. Infection can alter the results of the scan and make it invalid.
  • Drink lots of clear fluids and urinate before the test to keep your body hydrated.
  • Your doctor may ask you to avoid certain medicines prior to the test.
  • Certain questions may be asked about the pattern of your menstruation cycle if you are a growing girl.
  • They may also ask if you are pregnant or expecting one.
  • A small amount of DMSA will be injected into the patient’s vein and will be asked to wait for 2 hours prior the test.
  • In some cases, if the patient is a child or mentally challenged, your technician may inject a small dosage of sedative (anesthesia) to avoid movement during the scan. It is also used to calm the pulses if the patient is nervous or scared.
  • You can eat or drink before the scan if you have not been injected sedation.

How is the DMSA test performed?

Before the scan:

You will be taken to a nuclear medicine room and will be asked to lie on an examination table attached to Gamma rays (ionizing radiations) detecting machine.

The technician will inject a small dosage of radioactive tracer into a vein through a syringe. Sometimes, the scan is done immediately after the injection is given to the patient to evaluate the damaged areas of the kidneys or to check whether it is making urine normally. Else you will be asked to wait for 2-4 hours as your body needs time to absorb the radiotracer.

During this time you will feel no effect from the injection and can practice normal activities or diets.

During the scan:

After your waiting period gets over, you will be asked to lie on the examination table. A gamma camera will be positioned above the table which will take images showing the functioning of the kidneys. The gamma rays emitted by the tracer will be highlighted and seen under the gamma camera.

These imaging process can include one or several still images, lasting for 8-10 minutes. If the machine has the capability of creating 3D images where the camera moves around your whole body taking pictures from several angles. Then it may consume less time to give accurate results.

You will not feel any difference while being imaged neither there will be any noise or lights. Avoid movements during the scan as it can produce distorted images.

The whole procedure is done in two sessions. First is when the patient is given the radioactive tracer and second is when the patient is scanned for about 30-45 minutes.

After the scan:

The radiologist will remain in the room for most of the time. Your radiologist may ask for a second round of scanning if he doesn’t get convinced with the accuracy of your images.

You can leave the lab as your test gets over. There is no time required for recovery unless the patient has been given a dose of anesthesia.

A report of your scan will be available within 24 hours after the test.

Are there any risks associated with DMSA Scan?

  • The radioactive chemicals used in the scans are considered to be safe and flush out easily while urinating. The dose of radiation that your body receives is very small.
  • However, it may affect unborn children. So make sure to inform your doctor if you are expecting a child.
  • Some people may have an allergic reaction to the injected chemical.
  • There are chances that the patient may receive an overdose of the chemical in rare cases.
  • There may be a small amount of DMSA left in the urine for 24 hours after the test. Wash your hands with water and soap whenever you pass urine.
  • Pregnant women should avoid direct contact with their child’s urine or feces for at least 24 hours.

What are the possible interpretations of a DMSA Scan?

Your doctor is in the right position to interpret your results. However, the emitted gamma rays from inside the body are detected under the gamma camera. They are converted into an electrical signal and reflected on an attached computer.

The computer creates a picture by converting the intensities of radioactivity emitted into different colors or shades of grey. Areas of the target organ or tissue which emit lots of gamma rays may be shown as red spots on the visuals produced by the computer.

Areas that emit low levels of gamma rays may be shown as blue spots. Various other colors may be used to denote ‘in-between’ levels of gamma rays emitted.

Your doctor may ask you to do additional tests that may help him to assess your kidney problems. These tests include DTPA Renal Scan, Ultrasound KUB, Ultrasound KUB with PVR, KFT (Kidney Function Test), Urine Culture And Sensitivity, and Urine Routine Examination.
