What is MCU Test?

An MCU is an X-Ray test of the bladder usually conducted by a radiology lab or a hospital’s imaging department. MCU full form stands for Micturating Cysto-Urethrogram.

This test shows the bladder and the urethra passing urine which enables the doctor to find out if the urine goes straight to the kidneys from the bladder instead of going out through the urethra.

How is an MCU Test performed?

An MCU test procedure takes place in a room with an X-Ray table and a large camera above. We assume here that the test is for a child. Almost similar procedure will apply for adults. You may be allowed to stay in there with your child.

As for the procedure, the doctor cleans your child’s groin with antiseptic solution and a local anesthetic gel is applied to the end of a tiny tube called a catheter, which is then passed into the bladder. This procedure might cause discomfort to your child. Hence, it is important to reassure and comfort your child.

Then, the catheter is connected to a bag of contrast liquid. As the bladder fills, the dye allows the image of the bladder to appear on the TV screen in the shape of a black balloon. A series of images are then taken in different views. The same is repeated as the bladder empties.

It usually takes about 5-10 mins to prepare a child for the examination. While it might take a few minutes to perform the test in younger children, it might take upto thirty minutes in older children.

This test is generally performed on children under six months of age. An MCU test for adults may be done in case of recurring urinary tract infections and kidney damage. In adults or older children, an indirect nuclear medicine can be injected into the arm instead of a catheter being inserted into the bladder.

This way of testing could be a little less sensitive when compared to a radiographic MCU, but appropriate for children for who insertion of a catheter is more stressful than an injection.

How to prepare for an MCU Test?

No specific preparation is required for the MCU test procedure. You may want your child to be dressed comfortably before the test. Some children may also require sedation before the test. In such cases, it is recommended for your child to not eat or drink two hours before the examination. It is a great idea to consult your doctor or the lab for the same.

Carrying comforters, toys or stories to read with you might help your child to calm down before the examination. A dummy (pacifier) for babies can be helpful and soothing. Distracting your child and helping them focus on something that interests them might help in the smooth flow of the procedure.

If your child is old enough to understand his medical condition and examination, make sure you talk to him/her and explain the procedure in detail. This will help curb the anxiety. For your child to be more comfortable during the entire procedure, it is important for you as a parent/ guardian to allay your fears before you attend your own child.

What can be the possible interpretations of an MCU Test?

The images of an MCU are usually obtained by a radiologist who does the study with the help of a radiographer. A radiologist examines and interprets the X-Ray films, after which a written report is given to you. You will not be able to get access to the results immediately as studying the images take time. The time is taken to get the results of the test may depend on a number of factors, which include

What are the benefits of an MCU Test?

A Micturating-Cysto-Urethrogram shows images of the bladder as it fills up and empties. It helps doctors find out the causes of recurring Urinary Tract Infections and dilation of kidneys.

The entire aspect of an MCU is to find out abnormalities such as Vesico Ureteric Reflux (VUR), Posterior Urethral Valves (PUV), Hydronephrosis and Kidney Stones.

VUR is one of the most common triggers of UTI in children. In humans when normally observed, urine passes from kidney to the bladder. However, if suffering from VUR, the urine goes upstream, towards the kidneys.

PUV is a valve like a blockage at the bladder outlet. Infant boys can get this infection right from birth. An endoscopic surgery may be required to get rid of the blockage.

Hydronephrosis is a condition in which there is blockage higher up near the kidneys, which results in urine logging.

Like adults, children can also develop Kidney Stones at a very young age. 

This test can also help identify the abnormalities of the urethra.

Is there any side effects of an MCU Test?

There usually are no after or side effects from the procedure, but, the MCU test procedure could carry with itself the risk of a Urinary Tract Infection or an infection in the bladder as it involves insertion of a foreign object into the body. Your doctor might prescribe your child antibiotics around the time of the test to prevent such complications.

Some children may also feel a slight burning sensation while urinating for a short time after the examination (usually a day or two). But drinking plenty of liquids will help them get rid of it very quickly.

Although very rare, some children might struggle to empty their bladders. Getting them to urinate while sitting in a warm bath is the best way to solve this.

In case your child is taking any other medication, you don’t have to worry. The dye will not interfere with them and no extra precautions need to be taken.
