What is the ACE Test?
Angiotensin-Converting Enzyme is the full form of ACE. ACE is an enzyme that is responsible to regulate the blood pressure in our body. It converts angiotensin I to angiotensin II. Angiotensin-converting enzyme test measures the level of ACE in the blood.
The increased level of ACE in the blood may lead to a medical condition called sarcoidosis. Sarcoidosis is a chronic disease where small nodules are formed under the skin or in organs causing enlargement of lymph nodes in various parts of our body.
These small nodules that form under the skin are called granulomas. This disorder will increase the level of ACE in the blood.
When is the ACE Test Done?
Normally, the angiotensin-converting enzyme test is done if you show symptoms of having sarcoidosis. It affects various organs such as lungs, skin, liver, heart, spleen, lymph nodes, etc. The symptoms of sarcoidosis are
- Pain in joint
- Rapid breathing
- Granulomas
- Red eyes
- Loss of appetite
- Bleeding in nose
- Night sweats
- Swollen lymph nodes

What is the Procedure of the ACE Test?
A nurse will put a tight elastic band around your arm to make your veins swell. Then the area from where the blood is to be drawn will be cleaned by a disinfectant liquid.
A needle will be then injected into the vein to draw the blood sample. After removing the needle, the puncture site will be pressed with a cotton ball to stop the bleeding. The collected blood sample will be then sent to the laboratory for further testing.
What does the ACE Test Result mean?
The ACE test measures the level of angiotensin-converting enzyme in the blood. The normal range of ACE varies depending on age and gender. The normal range of ACE is
- >2 years – 18 to 95 units/l
- 3 to 14 years – 22 to 108 units/l
- >15 years – 14 to 82 units/l
If the level of ACE is more than the normal range, it means that you may have sarcoidosis. This level may increase by more than 50% if you have active sarcoidosis.
Sometimes the ACE level will be normal even if you have sarcoidosis. Other factors that increase the level of ACE are cirrhosis, HIV, histoplasmosis, lymphoma, tuberculosis, diabetes, etc.
Your doctor may also ask you to take other tests along with the ACE test such as AFB test to know the cause of an increase in ACE level.
