Leukocyte Alkaline Phosphatase is the full form of the LAP test. The LAP score test measures the level of alkaline phosphatase and a group of enzymes. These enzymes are commonly seen in your bone tissues, kidney, liver, and bile duct.
It can also be seen in the placenta of pregnant women. It measures the level of alkaline phosphatase present in the white blood cells which are also known as leukocytes. This test is generally used to diagnose chronic myeloid leukemia (CML).
It is done if you have an elevated level of white blood cells to differentiate myeloid leukemia and other medical disorders. The level of alkaline phosphatase will be low in the white blood cells if you have myeloid leukemia.
Why is LAP Score Test Done?
LAP score test determines the level of alkaline phosphatase in the white blood cells to rule out myeloid leukemia. Your doctor will ask you to take a LAP test if you show any symptoms of having chronic myeloid leukemia or other medical conditions such as
- Thrombocytosis – a high level of white blood cells that is not caused by infection or cancer.
- Leukemoid reaction
- Aplastic anemia – a medical condition where your bone marrow produces only few blood cells.

Is there any Preparation Required for LAP Test?
LAP test is a simple blood test. There are no special preparations required for the test. However, you may be asked to fast for few hours prior to the test.
It is important to inform your doctor if you are taking any medications before the test. It is also recommended to wear half-sleeved cloth to the test for convenience.
What is the Procedure of the LAP Test?
The procedure of the LAP test is similar to that of any other blood test. A nurse will tie a tight band around your arm to make the veins swell. The area from where the blood is to be drawn will be cleaned with a cotton ball soaked in a sanitary liquid.
Then a needle will be injected into the vein to draw the blood which will be collected in a tube. After removing the needle, the puncture site will be pressed with a cotton ball to stop the bleeding. The collected blood sample will be then sent to the laboratory for further testing.
What Does the LAP Test Result Mean?
LAP score test results help to detect myeloid leukemia. The normal range of LAP is between 20 and 100. However, sometimes it may range from zero to 400. The high level of LAP in the white blood cells indicates the following medical conditions
- Leukemoid reaction
- Myelofibrosis
- Polycythemia vera
- Essential thrombocytosis
If the level of alkaline phosphatase is low then it may indicate
- Chronic myeloid leukemia
- Aplastic anemia
- Pernicious anemia
