Frequently Asked Questions About SHBG Test

What is the SHBG Test?

Sex Hormone Binding Globulin is the full form of SHBG. It is a protein produced by the liver. It helps to examine the cause for a low level of testosterone in men and excess production of testosterone in women. SHBG binds and carries certain hormones such as testosterone, dihydrotestosterone (DHT), and estradiol throughout the bloodstream.

The cells in your body cannot use the hormones at this state. In simple words, if the SHBG level is low in the blood, your body has more hormones available. On the contrary, your body may not have enough hormones if the SHBG levels are high.

SHBG test is also done along with other tests such as prolactin, estradiol, and luteinizing hormone to evaluate the existing balance of hormones in your body. This test helps to detect the measure the level of total testosterone in the blood.

Why is SHBG Test Done?

SHBG test is generally done to diagnose the abnormal level of testosterone in the blood. Your doctor will ask you to take the SHBG test if you show any of the following symptoms

Men

  • Acne
  • Mood swings
  • Gain in weight
  • Low sperm count
  • Fluid retention
  • Erectile dysfunction
  • Large breast tissue

Women

  • Gain in weight
  • Irregular menstrual cycle
  • Excess facial or body hair
  • Tenderness in breast
  • Bloating

This test is also done if your total testosterone test does not provide a complete picture of the problem.

Is there any Preparation Required for SHBG Test?

Sex hormone-binding globulin test is a simple blood test. There are no special preparations required for the test. You can have your normal diet and carry on your regular activities prior to the test.

If you are taking any medications, consult with your doctor prior to the test. It is recommended to wear half-sleeved cloth to the test for convenience.

What is the Procedure of the SHBG Test?

The procedure of the SHBG test is similar to that of any other blood test. A nurse will put a tight elastic band around your arm to make the veins swell. The area where the blood is to be drawn will be cleaned with a sanitary liquid.

A needle will be injected into the vein to draw the blood samples which will be collected in a tube. After removing the needle, a cotton ball will be pressed on the punctured site to stop the bleeding. The collected blood sample will be then sent to the laboratory for further testing.

What Does the SHBG Test Result Mean?

The SHBG test measures the amount of SHBG in the blood to determine the level of testosterone. The normal range of SHBG in males is between 10 and 57 nmol/l and in women, it is 18 to 144 nmol/l.

The high level of the SHBG test indicates that there is only less free testosterone available for the tissues. On the other hand, the high SHBG means that there is an excess of total testosterone available. The SHBG level will be high if you have liver disease, eating disorders, hyperthyroidism, low sex hormone production (hypogonadism), etc.

The low level of SHBG can be seen if you have obesity, Cushing disease, hypothyroidism, polycystic ovary syndrome, etc.
