What is an X-Ray RGU Test?
An X-Ray RGU (Retrograde Urethrogram) is done to look for any abnormality in the urethra (the terminal part of the urinary passage).
The urethra is a tube that connects to the urinary bladder and carries urine from the bladder to outside the body. In males, the urethra travels through the penis and also carries semen.
It is typically recommended for males to diagnose urethral trauma or urethral stricture (narrowing of the urethra caused by an injury or infection). The RGU helps to diagnose the nature of the injury. It is commonly used for operative planning as well as a follow-up procedure after urethral reconstruction.
The complete procedure is known as Urethrography, which is performed with the help of radiopaque contrast to be injected into the urethra. The procedure is completely non-invasive and gives more impressive results as compared to other diagnostic imaging techniques to examine the urethra, such as Urethroscopy, Urethroplasty, Uroflowmetry test, and Ultrasound.
An X-Ray RGU can also be called RGU, Urethrography, or Urethrogram.
Who is recommended for an X-Ray RGU Test?
Your doctor may suggest an X-Ray RGU if you have a poor urinary stream if you feel strain to pass urine, continuous dripping of urine, external injury to the genital region, the presence of abscess or fistula, and congenital abnormality in the urethra suspected by your doctor.
It is also recommended to male patients with a previous history of urethral stricture. They may have symptoms such as urinary urgency, urinary frequency, and poor bladder emptying. An RGU is often performed for the imaging and evaluation of the urethra after a surgical procedure.
What are the preparations required for an X-Ray RGU?
- Wear loose and comfortable clothes. Preferably wear clothes in a pair of separate lower and a kurta or shirt as the patient will be asked to expose their genital organ.
- Make sure to empty the bladder prior to the test (urinate).
- Do not wear any metallic ornament or accessories during the scan.
- Do not apply any cosmetics or ointments to the genital area.
- Inform the concerned physician if the patient is suffering from a Urinary tract infection.
- Inform the doctor if the patient has any implanted metallic instrument in the body such as metal hip bones or joints.
What is the procedure for an X-Ray RGU?
The patient is taken to the scan room and asked to lie on an examination table. The patient will be asked to lie in a supine position (horizontally) with a slightly tilted leg position. They will be positioned under the X-Ray beam and asked to expose their genital area. The technician cleans the genital organ with an antibacterial liquid.
A small plastic tube called a catheter is inserted into the tip of the penis and the operator pulls the penis laterally to straighten the urethra. A small balloon is inflated to keep the catheter in position. Then a small amount of saline water is injected through the urinary outlet, in the reverse direction to the normal urine flow. The x-ray dye is then introduced into the urethra in a small dosage of 20-30ml which looks opaque on x-rays.
The patient may be asked to lie in different positions to demonstrate the urethra completely. The X-ray beams will be directed on the pelvis with the center focused just below the pubic bone. Occasionally, depending on what the doctor is investigating, the technician may fill the patient’s bladder with the help of the catheter and obtain further images during voiding. Static images of the urethra are taken during retrograde injection of radiopaque contrast. The examination should be complete in approximately 30 minutes.
Overfilling must be avoided, or it can rupture the distal urethra. (A conscious patient can be asked to alert the operator if he faces any pain during balloon filling).
What are the possible interpretations of X-Ray RGU?
Typically the X-Ray reports are available in a translucent template in gray and white color. It is quite difficult to understand for an individual if they are not formal doctors or radiologists. Your concerned doctor is in the right position to interpret your result.
The most common indication for an RGU is in the setting of trauma. The presence of blood at the urethral opening while penetrating the radiopaque contrast indicates an injury or infection. Penile fracture can also be an indication through a RUG. Another relative indication for a RUG is the finding of a “floating prostate” while examining, which may indicate urethral disruption. The prostate is a very important part of a male genital area. It is a wall nut-sized gland that produces seminal fluid. The normal prostate feels fixed on digital examination but if it is mobile or floating, it may be a sign of severe disruption in the urethra.
What are the risks of an X-Ray RGU?
- Due to the injected sedation, you may have delayed recovery after the test. You may feel dizzy all day long.
- You may feel a burning sensation while passing urine.
- You may pass blood while urinating once or twice after the test.
- If this continues then you must immediately rush to your concerned doctor.
- If you develop unusual and sudden fever, genital pus, or scrotal pain then there is something you must take notice of and inform your physician.
- If the contrast solution isn’t injected properly then it may create issues in the vascular system.
- Some patients may have an allergic reaction to the radiopaque (contrast dye).
- This procedure should not be performed with patients who already have an active urinary tract infection.
- When excessive pressure is applied while instilling contrast into the urethra, it may spill on the surrounding tissue causing discomfort to the patient in the future.
- This complication can be easily prevented by applying only enough pressure to fill the urethral.
- If penetrating trauma has injured the urethra while performing an RGU then leakage of urine often occurs.
