What is the TB gold Test?
QuantiFeron TB Gold Test is the full form of QFT-G. Mycobacterium tuberculosis is detected with the help of the QFT-G test. This bacteria is the main cause of tuberculosis (TB). It normally infects the lungs but also can spread to the kidney, brain, and spine. The QuantiFeron TB test is a controlled laboratory test.
The QFT-G is an aid in diagnosing Mycobacterium tuberculosis infection. It also helps to diagnose latent tuberculosis infection and tuberculosis disease.
What is tuberculosis?
Mycobacterium Tuberculosis causes TB which often affects the lungs. TB spreads easily from one person to another through the air. TB infected person who is not ill yet cannot transmit the disease. This is called latent TB. These bacteria grow slowly within our body cells by showing mild symptoms like cough, fever, weight loss, etc.
What are the symptoms of tuberculosis?
The TB bacteria normally grows in the lungs. It causes symptoms like
- Chest pain
- Coughing blood
- Cough may last for 3 weeks or more
- Sweating at night
- Fever
- Loss in weight
- Decrease or loss of appetite
- Chills
- Weakness
- Fatigue
Who are at high risk?
Tuberculosis is an air transmitted disease. A person can get affected by inhaling only few germs present in the air. The people who are at high risk of TB infection are
- HIV infected patients
- Old people with the weak immune system
- Immigrants from areas of the high incidence of TB
- Transplant patients
- Head and neck cancer patients
- Drug abusers
- Silicosis (a respiratory condition caused by inhaling silica dust)
What are the benefits of the TB gold test?
- Results are not subject to reader bias
- The booster phenomenon does not affect the result
- BCG vaccination does not affect the accuracy
- Requires only one patient visit
What are the limitations of the TB gold test?
- Does not differentiate between latent and active TB infection
- Available of limited data to determine who is at risk of developing TB infection
- Incubating blood samples within 12 hours of collecting them with antigens
How is the procedure performed?
The QuantiFeron TB gold test is a whole-blood test done to detect mycobacterium tuberculosis infection. The QFT uses three unique test tubes that enable immediate exposure of viable blood lymphocytes.
Each test tube contains three different TB antigens called ESAT-6, CFP-10, and TB-7.7(p4). The blood is collected in all three test tubes. Nil tube (grey), TB antigen (red) tube and Mitogen control (purple) tube are the three test tubes.
The blood collected in each tube should be exactly 1 ml. More or less than 1 ml may cause erroneous results. The test should not be performed if the samples did not arrive within 8 hours of collecting blood. The samples collected should not be refrigerated.
What does the result mean?
The standard ELISA format measures the amount of gamma interferon produced by each tube. The enzyme-linked immunosorbent assay is the full form of ELISA. It uses antibodies and color change to identify a substance. The QFT-G test results are based on the interferon-gamma released in response to the antigens present in each test tube.
Nil or negative control tube: This tube must have a value of ≤0.8 IU/ml in order to be a valid test.
Mitogen or positive control tube: The mitogen tube should have an interferon-gamma value of ≥0.5 IU/ml greater than the nit tube for a valid test.
TB antigen: The nil tube value minus the TB antigen tube value should be ≥0.35 IU/ml for a test to be considered positive. A positive test result likely suggests the presence of mycobacterium tuberculosis.
